Matching the service to the format 

Not every kind of psychiatric care travels equally well over video. Here’s the honest breakdown. 

Works well over telehealth 

Medication management. Initial evaluations, dose adjustments, side-effect troubleshooting, and ongoing follow-ups. Most people managing depression, anxiety, bipolar disorder, or ADHD medication can do the entire arc of care remotely. 

Follow-up and monitoring. Short check-ins are arguably better remote — a 15-minute appointment shouldn’t cost you two hours. 

Requires in-person visits 

TMS therapy. Transcranial magnetic stimulation delivers magnetic pulses to a mapped area of the brain, five days a week for four to six weeks, in sessions of roughly 20 to 40 minutes. It requires a chair, a device, and a technician. NE Portland patients currently do this at the SW Portland clinic. 

Esketamine (SPRAVATO®). Self-administered nasal spray, but federally required to be given in a certified clinic with two hours of monitoring afterward. Also in-person by necessity. 

ADHD testing. Some components can be done remotely; the full evaluation is typically in-office. 

Insurance in Oregon: what to check before you book

Three things worth knowing specifically about Oregon coverage: 

Telehealth parity. Oregon requires most commercial plans to cover telehealth behavioral health visits comparably to in-person visits. You generally aren’t paying more for the convenience. 

Advanced treatments need prior authorization. TMS and esketamine are covered by most plans for patients who meet criteria, but coverage is conditional — typically on a major depressive disorder diagnosis and documented inadequate response to a certain number of antidepressant trials. This is paperwork, not a wall, and our team handles the submission.

Oregon Health Plan is different from commercial coverage. If you’re on OHP, coverage for specialty psychiatric services depends on your coordinated care organization. Call and ask before you assume either way. 

How to actually start this month 

  1. Decide format before provider. If you need TMS or esketamine, you’re going in person and the location question is settled. If you need medication management, telehealth is likely the faster path to a first appointment. 
  2. Have your history ready. Every medication you’ve tried, roughly when, roughly what dose, and why you stopped. This single list is the biggest determinant of how productive a first appointment is — and if you’re pursuing TMS or esketamine, it’s also what the prior authorization is built from.

If your depression hasn’t responded to medication 

A meaningful share of people who’ve been on antidepressants for years are still symptomatic, and many were never told that’s a recognized clinical situation with its own treatment pathway. 

Treatment-resistant depression generally means depression that hasn’t improved adequately after two or more antidepressant trials at adequate dose and duration. It’s the entry point for TMS and esketamine, and both are covered by most insurance plans for patients who qualify.
